Job Description
We are seeking a full-time Nurse Practitioner or Physician Assistant to provide primary care in Skilled Nursing and Assisted Living Facilities in Portsmouth, NH, and surrounding areas.
Perform primary care visits, medication reconciliations, and annual wellness exams 4-5 days a week.
Examine patients, review medical records, order and interpret diagnostic tests, diagnose conditions, and prescribe treatments and medications.
Educate patients and families on health conditions, prevention, and management.
Maintain accurate records, promote health practices, and oversee overall patient health.
