Job Description

We are seeking a part-time, in-person provider for short, flexible daily shifts (7AM-1PM) with no on-call or overnight duties.

Key responsibilities:

  • Manage clinical care and rounding at skilled nursing facilities
  • Prescribe medications and treatments
  • Provide patient-centered primary care services
  • Ensure timely documentation using electronic health record systems
  • Communicate with nursing staff via phone or secure messaging

    • Requirements: Reliable transportation, punctuality, and ability to perform essential duties. Equipment and practice expenses are covered, with opportunities for training.
