Job Description
We are seeking a dedicated Full-Time Nurse Practitioner or Physician Assistant to join our Wound Care/Home Health team in Centralia, WA.
Work involves evaluating and managing acute, chronic, surgical, and dermatologic wounds in skilled nursing facilities. Perform wound dressings, debridements, and G-Tube procedures (training provided), establish care plans, and work within facility protocols. Opportunities include Home Health visits as needed. Must be outgoing, energetic, and experienced with SNF environments. Preferred skills include wound care certification and 2+ years of wound care, skilled nursing, and debridement experience. Requires strong communication, independence, current WA license, and certification. Benefits include health/dental insurance, flexible schedule, and paid time off. Schedule is day shift, Monday to Friday, with on-call and weekend duties as needed.
