Job Description
We are seeking a full-time Nurse Practitioner or Physician Assistant to join our Wound Care/Home Health team in Centralia, WA.
Provide wound assessments and management during weekly bedside rounds in skilled nursing facilities. Perform dressing changes, medically necessary debridement, and G-Tube procedures (training provided). Establish care plans and work within facility protocols, with opportunities for Home Health visits. Must have at least 2 years of wound care and skilled nursing experience, with strong communication skills and the ability to work independently. Current WA medical license required; wound care certification preferred. Benefits include health/dental insurance, paid time off, and flexible scheduling. Compensation ranges from $125,000 to $180,000 annually, depending on experience.
