Job Description
We are seeking a full-time Nurse Practitioner or Physician Assistant to join our Wound Care/Home Health team in Centralia, WA.
Provide wound evaluations, management, dressing changes, and debridement in skilled nursing facilities. Participate in weekly bedside wound rounds with treatment nurses. Perform G-Tube replacements/removals (training provided) and conduct Home Health visits as needed. Work independently within facility protocols and establish plans of care. Ideal candidates are outgoing, energetic, and have at least 2 years of wound care and skilled nursing experience. Must have current Washington license, strong communication skills, and familiarity with skilled nursing environments. Preferred certification in Wound Care; experience with debridement is required. Benefits include health/dental insurance, flexible schedule, and paid time off. Salary ranges from $125,000 to $180,000 annually. Schedule is Monday to Friday, day shift, with on-call and weekend duties as needed.
