The Medical Social Worker (MSW) provides psychosocial assessments and supports patients and families receiving Home Care and Hospice services. As part of the interdisciplinary team, the MSW offers resources for illness navigation, long-term care planning, and community connections.

Responsibilities involve

• Conducting psychosocial assessments and developing intervention plans

Qualifications include

• Master’s degree in social work from accredited school
• Minimum one year healthcare experience
• Strong communication skills, valid driver’s license, reliable transportation
