Breakwater Commons, a 96-bed brand new state of the art healthcare facility. This is a one of a kind facility boasting private living spaces for all residents, expansive therapy gym and panoramic views of Penobscot Bay is seeking a dynamic and customer service oriented individual to join our social services department.

Responsibilities will include but not limited to assisting with the skilled patient admissions process, providing support to residents and family members towards discharge planning and preforming a variety of social service & administrative duties.

The successful candidate will be a licensed social worker with a health care background and have good organizational and communication skills, be able to work independently and handle sensitive, urgent & confidential matters.

This part time position will offer flexible scheduling.
