Job Description

Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living facilities.

  • Deliver face-to-face primary care services, participate in educational sessions, and collaborate with healthcare teams.
  • Serve a range of patients from short-term rehabilitation to complex geriatric cases in skilled nursing settings.
  • Contribute to improving clinical, cultural, and financial outcomes through a structured care model.
