Job Description

Community Health is a primary care network offering comprehensive programs including wellness, dental, behavioral health, and pediatric specialties, serving a diverse patient population across Rutland and southern Addison counties.

We seek a Master’s Level Licensed Counselor to provide mental health treatment to children and adults in the Southern-Central Vermont area. The role involves individual, family, and group therapy using evidence-based techniques, developing personalized treatment plans, and collaborating with interdisciplinary teams to support clients’ emotional, relationship, and community integration goals.

Key functions include maintaining accurate records, facilitating referrals, administering assessments, and providing employee assistance upon request. Applicants should hold a Master’s Degree and be licensed (e.g., LMHC, LICSW), with strong interpersonal, communication, and EMR skills, and a client-centered, recovery-oriented approach.
