We are seeking a full-time Primary Medical APP to join our innovative post-acute care team, focusing on an interdisciplinary approach.
Manage acute and chronic patient conditions through examinations, diagnostics, treatment plans, and referrals. Provide patient and family education, handle triage calls, and maintain detailed medical records. Collaborate with team members and participate in facility metrics meetings to improve outcomes. Enjoy a supportive work environment with no call duties, flexible schedule (3-day workweek, 8 AM–5 PM), and occasional remote documentation. Require active PA or NP license and at least two years of relevant experience, preferably in rehabilitation settings.
