Requirements:

• Unencumbered Illinois Nurse Practitioner license
• Master’s Degree in Nursing
• Preferred 2+ years of experience
• Strong communication, organizational, and time management skills

Responsibilities:

• Collaborate with patients and multidisciplinary teams to develop treatment plans
• Conduct assessments, diagnoses, and treatments based on evidence-based guidelines
• Create weekly rounding schedules for facilities
• Maintain accurate medical records
