Job Description

We offer in-home and virtual health assessment roles for Nurse Practitioners and PAs, focusing on delivering personalized care to members in their homes.

Our providers conduct comprehensive evaluations, including medication reviews, physical exams, and health discussions, to improve clinical outcomes and support community health.

Benefits include competitive pay ($100-$120/hour), flexible scheduling (Monday-Sunday, 8am-8pm), mileage reimbursement, completion bonuses, and employer-provided malpractice and collaborative physician support.

Qualifications include an active, unencumbered nursing license, board certification from AANP or ANCC, current CPR, and experience with elderly and chronically ill patients. Knowledge of Medicare/Medicaid and excellent communication skills are preferred.
