Job Description
The Family Medicine Physician, supervised by the Lead Physician and Chief Medical Officer, provides comprehensive primary and urgent care to a diverse patient population, ensuring high-quality services aligned with protocols and policies.
- Leads the care team using the Patient Centered Medical Home model, performing examinations, assessments, and developing treatment plans for patients of all ages.
- Delivers care via in-person, telehealth, or phone, ensuring documentation meets legal standards.
- Manages acute and chronic conditions, promotes preventive health, and educates patients on health and family planning.
- Participates in after-hours call rotation, follows up on urgent cases, and coordinates patient referrals.
- Ensures compliance with regulatory standards, maintains accurate records, and participates in quality improvement initiatives.
- Collaborates effectively with staff, communicates clearly, and fosters positive relationships with outside agencies.
- Practices cost-effective medicine, completes timely documentation, and supports clinic operations.
