Job Description
We are seeking a compassionate *Speech Therapist (ST)* to provide patient-centered care in a home health setting.
Evaluate, diagnose, and develop personalized treatment plans for speech, language, communication, and swallowing disorders. Deliver therapy interventions in patients’ homes and collaborate with healthcare teams to ensure coordinated care. Complete accurate clinical documentation and educate patients and families on treatment goals and exercises. Maintain compliance with regulations and agency policies. Requirements include an active Speech-Language Pathologist license, at least 1 year of home health, geriatrics, and cognition experience, reliable transportation, and willingness to relocate to Brighton, MI.
