The Family Medicine Physician provides comprehensive primary and urgent care to a diverse patient population under supervision, ensuring high-quality services aligned with clinical protocols and policies.
Leads the care team using a Patient Centered Medical Home model, performing examinations, assessments, and developing treatment plans for patients of all ages.Delivers care through in-person, telehealth, and phone modalities, ensuring proper documentation and compliance with legal standards.Manages acute and chronic conditions, screens for health maintenance, and educates patients on prevention and family planning.Participates in quality improvement, ensures regulatory compliance, and maintains accurate records and documentation.Fosters effective communication with staff, patients, and external agencies, promoting teamwork, confidentiality, and respectful relationships.Contributes to administrative tasks, supports clinic programs, and practices cost-effective medicine.Requires BC/BE, valid medical license, DEA, NPI, and ability to work in North Hollywood, CA, with a salary range of $275,000–$290,000 annually, offering comprehensive benefits. Spanish proficiency is preferred.
