We are a growing provider of innovative medical care for long-term and post-acute patients across the U.S., utilizing advanced technology to identify at-risk individuals and deliver timely care.

We seek a full-time on-site Nurse Practitioner or Physician Assistant to provide care in Skilled Nursing Facilities in Corsicana, TX. No weekends, nights, calls, or holidays required.

Responsibilities include conducting health assessments, developing care plans, managing chronic and acute conditions, ordering tests, prescribing treatments, and collaborating with a multidisciplinary team. The role also involves patient and family education, documentation, and participating in quality initiatives.

Requirements: Current Texas licensure and board certification, geriatric experience preferred, strong assessment and communication skills, proficiency with EMR systems, and ability to travel occasionally.
