We are a rapidly expanding provider of innovative medical care for long-term and post-acute patients across the U.S. utilizing advanced technology to proactively manage patient health and reduce hospitalizations.

We are seeking a full-time on-site Nurse Practitioner or Physician Assistant to deliver medical care at Skilled Nursing Facilities in Corsicana, Texas. This role offers a weekday schedule with no nights, weekends, or holidays, allowing for independent work supported by a collaborative team.

Primary responsibilities include conducting health assessments, developing care plans, managing chronic and acute conditions, interpreting diagnostic tests, prescribing treatments, and providing resident education. Collaboration with interdisciplinary teams and participation in quality initiatives are essential.

Requirements include current Texas licensure and board certification as a Nurse Practitioner, with preferred experience in geriatrics or related fields. Strong assessment, communication, and EMR skills are necessary. Occasional travel within the region may be required.
