Job Description
We are seeking a full-time Nurse Practitioner (NP) to provide compassionate hospice, palliative, and house call care in the Treasure Valley area.
Work involves collaborating with a multidisciplinary team to deliver personalized care to patients at home or in care facilities, focusing on advanced care planning and symptom management.
Responsibilities include conducting face-to-face visits to meet regulatory requirements and supporting aging in place.

Qualifications: Valid NP license in Idaho and/or Oregon, board certification (ANCC or AANP), valid driver’s license, and willingness to obtain licensing for both states if only licensed in one.
This in-person role requires relocation to Meridian, ID, and involves travel (preferred 75%).
