Job Description
We are seeking a Licensed/Board Certified Nurse Practitioner – Wound Care to provide in-home care for patients with acute and chronic wounds.
Provide in-person evaluations and management visits in home, assisted living, or skilled nursing facilities, focusing on wound healing and skin disorders.
Perform necessary debridement and procedures within CMS guidelines.
Follow infection prevention protocols and accurately document services in EMR for billing purposes.
Use critical thinking to assess, diagnose, and develop care plans, including prescribing treatments and medications.
Requirements include a Master’s Degree, Montana RN license, Nurse Practitioner certification, WOCN wound care certification (or within 90 days), at least 3 years of wound care experience, and valid driver’s license.
