Job Description
We are seeking a Licensed/Board Certified Nurse Practitioner to provide home-based wound care for patients in a multi-specialty practice.
Perform in-person evaluations and management of acute and chronic wounds in home, assisted living, or skilled nursing facilities, with about 80% of time dedicated to direct patient care.
Conduct medically necessary debridement and procedures within CMS guidelines to promote healing.
Follow infection prevention protocols and handle contaminated equipment appropriately.
Accurately document services in EMR to ensure proper billing according to CMS standards.
Use clinical judgment to assess, diagnose, and prescribe treatments and medications for wound-related conditions.
Must have a Master’s Degree, Montana RN license, Nurse Practitioner certification, and WOCN (or be willing to obtain within 90 days). A minimum of 3 years as a wound specialist and 1 year as a Nurse Practitioner in wound care is required.
