Job Description

Seeking an energetic Nurse Practitioner for a full-time wound care rounding role in Portland, OR, focused on positive patient outcomes and team collaboration.

  • Manage a patient load across Skilled Nursing Facilities, assisted living, and home visits.
  • Provide direct patient care, implement treatment protocols, and ensure accurate documentation.
  • Collaborate with clinical leadership to improve care quality and meet compliance standards.
  • Communicate effectively with patients and healthcare partners, and coordinate care activities.
  • Maintain certifications, including wound care credentials within one year if not already certified.
  • Requirements include board certification, master’s degree, wound care experience, and independent work capability.
  • Travel in personal vehicle with mileage reimbursed; opportunity for salary up to $180,000/year and benefits including health, dental, vision, 401(k), paid time off.
  • No weekend shifts; full-time, Monday-Friday schedule.
