Growing wound care company in Portland, OR seeks an energetic Nurse Practitioner for a full-time SNF Rounding Clinician role.
Manage a full patient load, providing direct care in skilled nursing facilities, assisted living, and patient homes.
Implement treatment protocols, document care accurately, and coordinate with the clinical team to enhance patient outcomes.
Communicate effectively with patients, families, and healthcare partners, ensuring timely follow-up and chart reviews.
Maintain certifications, credentials, and adhere to company protocols. Obtain wound care certification within a year if not already certified.
Requirements include board certification, 1+ year wound care and NP experience, and ability to work independently. Travel required with mileage reimbursement.
