Growing wound care-focused company in Portland, OR seeks an energetic RN Practitioner for full-time patient rounding in skilled nursing facilities and residential settings.

  • Manage a full patient load, providing direct care, implementing treatment plans, and documenting all care accurately.
  • Work closely with clinical leadership to improve care quality and ensure compliance with protocols and documentation standards.
  • Responsibilities include coordinating patient visits, responding promptly to communications, attending meetings, and maintaining certifications.
  • Requires Board-Certified Nurse Practitioner with 1+ year wound care experience and relevant certifications (WCC, CWS, WOCN preferred). Must be independent, communicative, and willing to travel.
