Primary Care Functional Medicine Nurse Practitioner or Physician Assistant

Job Description

We are seeking a Primary Care Functional Medicine Nurse Practitioner or Physician Assistant to deliver personalized, integrative primary care in clinics across Oregon.

Provide patient care via office visits, telehealth, and same-day appointments, including assessments, diagnostics, and treatment planning.

Develop individualized plans incorporating lifestyle, supplements, hormone therapies, and prescriptions.

Educate patients on preventive and functional medicine principles while collaborating with a multidisciplinary team.

Document visits accurately in the EHR and participate in team and quality improvement activities.

Qualifications: Active Oregon license, board certification, 2+ years of clinical experience, strong communication skills, and proficiency with EHRs.
