Job Description

*Position Summary*

We are growing — and seeking a *Part-Time Family Nurse Practitioner (FNP)* who is passionate about *community-based care* and *mobile health outreach*.
Under the direction of the Director of Nursing and in collaboration with the Health Department’s Medical Director, the FNP will deliver *primary and behavioral health services* that advance the *OneOhio opioid settlement goals* and expand access to care for vulnerable populations.

This role combines clinic-based services with *mobile outreach* — providing screenings, vaccinations, treatment, and education directly to individuals in need, including those affected by *substance use disorder (SUD)*, *homelessness*, and *limited access to care*.

*Key Responsibilities*

* Provide care in *clinic and mobile settings*, including the *mobile health unit*, community events, shelters, and outreach locations.
* Assess, diagnose, and treat acute and chronic conditions following public health and clinical best practices.
* Initiate and manage *Medication for Opioid Use Disorder (MOUD)* (e.g., Vivitrol) in accordance with DEA standards.
* Administer vaccines and conduct screenings for *Hepatitis C, HIV, STIs, TB*, and *pregnancy*.
* Provide *wound care*, *infection management*, and *preventive services* for individuals who use drugs.
* Deliver *reproductive health* and *family planning* services, including STI treatment and Nexplanon insertions.
* Distribute *naloxone (Narcan)*, educate clients on overdose prevention, and promote harm reduction practices.
* Collaborate with *peer navigators, case managers, and harm reduction staff* to ensure warm handoffs and continuity of care.
* Document all services accurately in electronic health systems (e.g., REDCap).
* Engage with community partners and participate in case management with public health, behavioral health, and social service agencies.

*Qualifications*

*Required:*

* Current *Ohio Certified Nurse Practitioner* license (FNP-C or FNP-BC)
* Valid *DEA license*
* Minimum *2 years of experience* in primary care, community health, or SUD services
* *BLS/CPR certification*
* Strong knowledge of trauma-informed and culturally competent care
* Valid *Ohio driver’s license*

*Preferred:*

* Experience with *mobile health outreach* and *community-based care*
* Familiarity with *MOUD* and public health programs
* Experience serving individuals impacted by the opioid epidemic

Job Type: Part-time

Pay: $48,000.00 – $49,920.00 per year

Expected hours: 16 per week
