Job Description

Seeking a dedicated Primary Geriatric Therapist to provide individual, group, and family therapy to adults in a nursing home setting as part of PHP/IOP programs.

Responsibilities include developing and implementing personalized treatment plans, ensuring regulatory compliance, participating in clinical meetings, facilitating meaningful activities, and maintaining accurate documentation in the EHR system.

Qualifications involve a Master’s degree in a mental health field, an active Texas license (e.g., LCSW, LPC, LMFT), and preferably experience with geriatric populations or PHP/IOP settings. Strong communication skills and familiarity with electronic charting are essential.
