We are seeking a friendly and dedicated Primary Care Physician to join a growing PACE Organization in Springfield, MA, on a full-time basis.

    • Under supervision of the Medical Director, the physician will provide comprehensive primary care to PACE participants, including assessments, care planning with the interdisciplinary team, direct patient care, and evaluation of care effectiveness. The role involves independent decision-making and performing medical procedures as needed.

  • Qualifications include Board Certification or Eligibility in Internal Medicine or Family Practice, with advanced geriatrics certification preferred. Knowledge of the needs of frail older adults is essential. Bilingual in Spanish is a plus.
