Responsibilities:
Manage adult patient panels, providing personalized chronic, acute, and preventive care.
Deliver preventive services, health screenings, and vaccinations.
Utilize data to support population health and care gap identification.
Collaborate with interdisciplinary teams for integrated care.
Maintain accurate EHR documentation and ensure compliance.
Drive practice innovation, embrace new tools, and meet clinical and financial goals.
Qualifications:
MD or DO with active US license.
Experience in adult primary care, chronic disease management, and interest in value-based care.
Excellent communication skills.
