Nurse Practitioner

Job Description

We are seeking a compassionate Nurse Practitioner for our adult primary care practice in the Champlain Valley, NY, near Albany, Burlington, Lake Placid, and Quebec.

Perform physical exams, diagnose, and treat acute and chronic conditions.

Develop and adjust treatment plans collaboratively with patients and healthcare team.

Provide health education, monitor progress, and ensure coordinated care.

Accurately document patient information using EHR systems.

Qualifications include a valid NY NP license or eligibility, experience in primary or urgent care preferred, strong communication skills, and proficiency with EHR.
