Nurse Practitioner

Job Description

We are seeking a compassionate Nurse Practitioner for our adult primary care practice in the Champlain Valley, NY, near Albany, Burlington, Lake Placid, and Quebec.

Perform physical exams, diagnose, and treat acute and chronic illnesses.

Develop and implement patient-centered treatment plans.

Provide health education, monitor progress, and collaborate with the healthcare team.

Accurately document patient information using EHR systems.

Qualifications include a valid NY NP license, preferred experience in primary or urgent care, strong communication skills, and proficiency with EHRs.
