Job Description
Seeking a full-time or part-time Nurse Practitioner or Physician Assistant for primary care in Skilled Nursing and Assisted Living Facilities in Keene, NH, and nearby areas.
Provide primary care visits, medication reconciliation, and annual wellness exams 3-5 days per week.
Examine patients, review records, order diagnostic tests, diagnose, and treat illnesses.
Prescribe medications, educate patients and families, and promote health practices.
Maintain accurate records and oversee patient health, with access to an online portal.
