Job Description
Seeking a full-time or part-time Nurse Practitioner or Physician Assistant to provide primary care in Skilled Nursing and Assisted Living Facilities in Keene, NH, and nearby areas.
Perform primary care visits, medication reconciliations, and annual wellness exams 3-5 days a week.
Examine patients, review records, order diagnostic tests, diagnose conditions, and prescribe treatments and medications.
Educate patients and families on health, promote wellness, and oversee overall patient health.
Maintain accurate records, schedule appointments, and utilize an online portal for updates.
