Job Description
The Hamilton Healthcare System *Licensed Clinical Social Worker (LCSW)* is responsible for day-to-day clinical operations of the Department’s Behavioral Health Counseling Services. This individual will also take part in the RHC provider collaboration to provide needed services to our population. OSHA Bloodborne Pathogens Class I.
*EDUCATION and EXPERIENCE REQUIREMENTS:*
* A Degree in Clinical Social Work *Required *
* Current LCSW License in the State of Texas is *Required*
* Experience in initial clinical assessment and admission work, discharge planning and patient chart development preferred
* Training or Experience in non-violent crisis intervention is preferred
* Experience in Trauma Informed Care Methodology is preferred
*QUALIFICATION REQUIREMENTS: *
* Must have good understanding of state and federal regulations that govern the practice of Social Work.
* Must have an excellent and expert understanding of the clinical group therapy process and related clinical dynamics.
* Must have excellent clinical documentation skills.
* Must exhibit expert communication and clinical skills. Must possess organizational awareness.
* Must have the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, clinical documents or governmental regulations.
* Must have expert decision making skills and critical thinking ability; must perform daily duties independently with little support.
* Must have the ability to define/solve problems, define complex problems, deal with a variety of concrete and abstract variables in situations where only limited standardization exists, collect data, establish facts, and draw valid conclusions.
* Should possesses the ability to interpret an extensive variety of instructions furnished in written, oral, diagram, or schedule form.
* Should possesses the ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
* Must be computer literate and possesses working knowledge of fax, copier, and scanner.
*Responsibilities*
* Provide individual therapy/ counseling services as scheduled.
* Assure all tasks performed are within span of control and scope of clinical license and in compliance with related State and Federal guidelines, rules and regulations.
* Ensure patient dignity and respect patient values. Operate within ethical standards.
* Assess and reassess with the treatment team the patient’s treatment needs.
* Develop, monitor, and evaluate the patient’s treatment plans and progress.
* Participate in the referral development process.
* Communicate effectively with families and agencies, conveying a positive, professional image. Provide resources and support for patients before, during, and after treatment.
* Ensure timely and accurate documentation of all assessments and progress notes.
* Communicate results and trends to leadership on ongoing basis regarding concerns, improvements, changes, etc. Identify areas needing improvement and utilize the facility performance improvement process as directed.
* Coordinates Department’s patient clinical outcome measurement process.
* Participates in Department-wide meetings to maintain two-way communication, problem solving, and information passing a minimum of once a month.
