Seeking a compassionate Master of Social Work (MSW) for a full-time, salary-based role in Springfield, MA, supporting elderly individuals to maintain independence and dignity.

  • Conduct psychosocial assessments and create personalized care plans.
  • Provide counseling, advocacy, and support to participants and families.
  • Collaborate with an interdisciplinary team for holistic care coordination.
  • Assist with crisis intervention, discharge planning, and resource referrals.
  • Ensure compliance with regulations and proper documentation.

    • Qualifications include an MSW, with preferred licensure (LCSW or LICSW), and experience with geriatric or long-term care populations is highly desirable. Strong interpersonal and organizational skills are essential. Bilingual abilities are a plus.
