*Job Overview*

The Integrated Care Center is a federally qualified health center that provide primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We provide care to those most in need including those who are suffering from homelessness, mental illness, substance use, and chronic medical conditions. We provide team based care in an integrated care setting. We are looking for a mission driven, Addiction Provider who works well in a team, has experience with a safety net population and champions quality care. The provider must also be versed in harm reduction and trauma informed care.

The Addiction Provider works out of our central intake department in San Francisco. This program is the gateway for clients entering our withdrawal management program and four residential Substance Use Disorder treatment programs. The Addiction Provider works collaboratively with a team of clinicians to provide safe and compassionate access to treatment for San Francisco’s most vulnerable adults.

Under the clinical supervision of the Director of Addiction Medicine, the Addiction Provider is primarily responsible for the following: 1) management of intoxication and withdrawal states, 2) the initiation and monitoring of Medically Assisted Treatment, 3) the evaluation and management of addiction related medical and psychiatric problems. 4) recovery support and counseling, and 5) referral and care coordination for other aspects of care.

The Addiction Provider is responsible for conducting medical histories, performing physical examinations, applying screening tools, ordering and interpreting laboratory tests, and completing standard documentation related to the treatment of addiction. They are responsible for working in collaboration with medical and behavioral health staff in our Central Intake Department, Withdrawal Management Program, and Residential and Outpatient Programs as well as coordinating care with our Medical clinics. They provide direct patient care within HealthRIGHT 360’s Central Intake Department, Withdrawal Management Program, and/or Residential Programs.

The Addiction Provider may also provide supervision for Advanced Practice Providers in Collaborative Agreements, providing input in the clinical review, performance appraisal, peer review and hiring and termination of such staff.

The Addiction Provider also supervises the MAT healthcare treatment team in the absence of the Director of Addiction Medicine.This supervision may include managing Medical Assistants and Nurse functions. The position may require after hours work in assisting Behavioral Health staff during the night and on weekends.

KEY RESPONSIBILITIES

_Skills & Medicine Responsibilities: _

* Determine severity of drug and alcohol use based on current DSM criteria.
* Evaluate and treat drug and alcohol related withdrawal with medications.
* Determine appropriate ASAM Level of Care based on ASAM criteria
* Assess clients for clearance to enter withdrawal management and prescribe appropriate medication to support withdrawal process.
* Prescribe medication assisted treatment to aid in recovery goals.
* Evaluate clients in Withdrawal Management and Residential Programs who return to using substances.
* Evaluate clients in Withdrawal Management and Residential Programs who return from the ED or Hospital due to an overdose or any other medical condition.
* Monitor clients in the Withdrawal Management Program to ensure safe and effective treatment.
* Assist clients with state disability insurance applications during the residential program treatment.
* Conduct physical examinations for clearance for the Residential Program
* Interpret Urinary Drug Screens
* Monitor clients in Residential Programming for medication assisted treatment compliance.
* Assist Health & Wellness staff with inquiries about medications.
* Educates clients on opioid reversal and Narcan Use.
* Analyzes and interprets patient history, symptoms, physical findings, and/or diagnostic information to develop appropriate diagnoses.
* Maintains complete and detailed records of medical history, assessment, and health care plans.
* Uses motivational interviewing or recovery coaching
* Consulting with the Director of Addiction Medicine for treatment or referral options.
* Collaboration with behavior health and psychiatry teams.
* Provide oversight for Advanced Practice Providers including entering into a Collaborative Agreement with the Advanced Practice Provider, providing supervision, having input in the clinical review, performance appraisal, peer review and hiring and termination of providers and clinical support staff.

_*Education and Experience*_ _*Required*_:

* M.D. or D. O. degree from accredited United States medical school.
* Successfully completed internship and residency at accredited United States Internship or Residency Program.

_*Certification/Licensure:*_

* Board Certified in family medicine or internal medicine.
* Board certified or registered in addiction medicine, or equivalent experience.
* Valid and active California medical license,
* Eligible to participate in the CMS Medicaid/Medicare reimbursement programs.
* Valid and active DEA license with X waiver to prescribe buprenorphine.

Job Type: Full-time

Pay: $260,000.00 – $290,000.00 per year
