*What You’ll Do*

* *Conduct Thorough Exams:* Perform comprehensive physical assessments and closely monitor patient condition changes.
* *Prescribe with Confidence*: Initiate appropriate medications and treatment plans tailored to each patient’s needs.
* *Shape Patient Care*: Develop and manage care policies while facilitating timely referrals to specialists.
* *Document & Communicate Clearly*: Maintain accurate, timely, and comprehensive records while keeping all team members informed.
* *Collaborate to Elevate Outcomes*: Work closely with physicians and interdisciplinary teams to enhance patient care and results.
* *Drive Proactive Care*: Identify and act on opportunities to prevent complications and reduce re-hospitalizations.

*What You Bring*

* *Licensure:* Current, unencumbered Nurse Practitioner license
* Master’s Degree in Nursing (accredited program)
* *Experience:* 2+ years preferred, with strong background in infectious disease, geriatrics, or adult medicine. (SNF, LTC, palliative, hospice)
* Excellent communication and interpersonal skills with elderly subacute and chronically ill patients
* Knowledge of Medicare, Medicaid, and health plan structures (preferred)
* Ability to travel between facilities 20-25 miles radius
