Established in 1998, this mobile primary care practice delivers healthcare in patients’ homes, focusing on vulnerable adult and geriatric populations.

*Position Summary*
Seeking a patient-centered *Psychiatric ARNP* to conduct evaluations, manage medications, and support behavioral health via *remote telehealth* with occasional in-person visits in King, Pierce, and Snohomish counties.

*Key Responsibilities*
– Perform psychiatric assessments and create treatment plans
– Manage medications and monitor behavioral health conditions like anxiety, depression, bipolar disorder, PTSD, and dementia
– Collaborate with primary care providers, caregivers, and team members
– Document care using ELation Health EMR, ensuring compliance
– Educate patients and caregivers on diagnoses and safety

*Qualifications*
– Active WA ARNP license with prescriptive authority
– PMHNP certification
– Experience with adult and geriatric patients, telehealth, and home-based care preferred
