Job Description

The Encore Health Partners Rehab Speech and Language Pathologist provides clinical evaluation and direct patient care in an assigned facility, with administrative duties as needed.

Qualifications include a Bachelors and/or Master’s Degree in Speech-Language Pathology, current state licensure, excellent communication skills, and knowledge of therapy principles, materials, and accreditation standards.

Responsibilities encompass screening, evaluating, and treating patients, developing and implementing treatment plans, supervising staff, maintaining documentation, participating in interdisciplinary and family meetings, and ensuring confidentiality and resident rights.

Additional duties involve staff training, adhering to safety protocols, fostering teamwork, maintaining customer service, and complying with facility policies and HIPAA regulations.

The role requires physical ability to perform transfers and lift patients, emotional resilience working with compromised residents, and cognitive skills to prioritize tasks. Candidates must be able to work independently, communicate effectively, and operate safely in a healthcare environment.

Work location is in Leitchfield, KY, with a flexible schedule and mandatory relocation before starting. The position is in-person, emphasizing safety, teamwork, and quality patient care.
