Job Description
Our small family practice clinic, located in Salem, has an opening for a Family Nurse Practitioner. If you desire to work in an environment with less than the average number of patient visits per day, giving the Provider more time to spend with patients and really get to know the patient and their needs, this position may be for you. Our clinic provides a relaxed, supportive environment for both staff and patients. This position can be Part-time (3-4 days per week, 24-32 hrs per week) or Full-time (5 days per week, 40 hrs per week), depending on what works best for your schedule.
Clinic Hours: Monday – Friday 8:00-4:30 (Possible expansion of hours in the future to include later evening if needed)
We offer In-Office and Telehealth Appts – depending on pt need

Provide continuous and comprehensive services necessary for the promotion of optimal health, prevention and treatment of diseases and chronic conditions, and health maintenance. This includes the assessment, diagnosis, and management of health problems and disorders. Our clinic serves children and adults. You will work with a talented group of Health Professionals who are dedicated to promoting quality care and an enjoyable work environment.
*Typical Duties & Responsibilities*
* General preventive care, women’s health, men’s health, acute and chronic health conditions, consultations, general medical practice.
* Average 8-15 patient visits per day, depending on visit types
* Perform a thorough patient evaluation, determine a differential diagnosis and prepare a plan for treatment.
* Manage and document patient care; meeting standards for periodic chart reviews.
* Adhere to standards via identified text and/or via office practice standard protocols.
* Communicate effectively to patient, support staff and coordinating care agencies.
* Functions as a member of an interdisciplinary team.
* Maintain a customer-oriented approach to meeting patient needs in an expedient manner, involving the family in the plan of care as appropriate.
* Maintain confidentiality and patient privacy.
* Seek consultations as appropriate
* Act as a consultant to other providers at the clinic

*Education/Experience/Certification:*
* Oregon Nurse Practitioner license with prescriptive privileges
* National certification
* DEA license

Job Types: Full-time, Part-time
Pay: $105,000.00 – $140,000.00 per year
Expected hours: 24 – 40 per week
