Home-Based Therapist

Job Description

We are seeking a compassionate Home-Based Therapist to provide in-home mental health services to children, adolescents, and families in Bad Axe, Michigan.

• Deliver clinical assessments, develop and monitor individualized family service plans, and conduct regular home visits to support family functioning and promote healthy development.
• Maintain consistent contact with families, act as a liaison with community agencies, and provide crisis stabilization during weekday hours and on-call coverage.
• Document progress, advocate for families, and participate in program planning and community initiatives.

Qualifications include a Master’s degree in Social Work, Counseling, or Psychology, Michigan licensure, at least 2 years of relevant experience, and a valid driver’s license. Flexibility for evening and weekend hours is required. The role emphasizes culturally competent, trauma-informed, and person-centered practices.
