Home-Based Therapist – Children & Families
Location: Bad Axe, Michigan -Full-time
We are seeking a compassionate and skilled *Home-Based Therapist* to provide intensive, in-home therapy services to children, adolescents, and their families. This role is designed to promote healthy child and adolescent development, strengthen family functioning, preserve and reunite families, and reduce reliance on psychiatric hospitals or substitute care settings.
What You’ll Do
* Provide written clinical assessments for children and families on your caseload
* Complete mandated level-of-care assessments at intake, quarterly, and closure intervals
* Develop, implement, and monitor individualized family service plans, adjusting as needed
* Conduct regular home visits to teach parenting skills, model behavior management, provide therapy, and support family relationships
* Maintain at least four hours of face-to-face contact per month with each family
* Act as a liaison with schools, DHS, Juvenile Court, and other community agencies
* Provide crisis stabilization services during weekdays (2:00–7:00pm) and participate in rotating on-call coverage evenings and weekends
* Document progress notes and ensure continuity of care
* Advocate for families by linking them to community resources such as Head Start, recreational programs, and medical consultants
* Participate in program planning, supervision meetings, and community education initiatives
Qualifications
* *Education:* Master’s degree in Social Work, Professional Counseling, or Psychology
* *Licensure:* Michigan registration as a Social Worker, Licensed Professional Counselor, or Psychologist (required)
* *Experience:* Minimum 2 years working with children and families preferred
* *Additional Requirements:*
* Must meet qualifications of CMHP and QMHP (or work under supervision of one)
* Valid, unrestricted driver’s license
* Willingness to complete Infant Mental Health endorsement during employment
* Ability to work flexible hours, including evenings and weekends

Job Type: Full-time
Pay: $29.17 – $39.48 per hour
