1099 MD, Initiating Patient Visits

Job Description

We are seeking a qualified MD/DO to establish patient relationships for chronic care management services across various locations, including rural areas.

• Availability for 8-14 days per month to conduct approximately 375 onsite and remote clinical visits.
• Accurately document patient encounters within our platform and provide daily progress reports.
• Must have reliable transportation and be able to travel within 1-2 hours of the assigned region.
• Require active, unrestricted medical licenses, excellent clinical judgment, and familiarity with EHR systems.
