Job Description
Seeking a licensed social worker for full-time, in-office hospice support in Fruitland, ID, primarily serving multiple counties.
Provide bio-psycho-social assessments, develop care plans, and coordinate with the interdisciplinary team.
Conduct bereavement assessments, manage caseloads, and facilitate grief and caregiver support groups.
Educate patients and families on advance directives, end-of-life care, and placement options; refer to community resources.
Assist with crisis management, documentation, and ensure compliance with regulations and policies.
Qualifications include a Master’s or Bachelor’s in Social Work, current licensure (LCSW, LMSW, LSW), and willingness to obtain Oregon licensure within 60 days. Hospice experience is preferred.
