Job Description

The Nursing Home Wound Care Advanced Practitioner (AP) is a full-time clinical professional, either a Nurse Practitioner or Physician Assistant with specialized wound management training, responsible for assessing, treating, and preventing wounds in long-term care residents.


  • Perform comprehensive assessments and develop individualized wound care plans.
  • Administer advanced wound treatments, including debridement and negative pressure therapy.
  • Collaborate with interdisciplinary teams to ensure integrated care.
  • Implement prevention strategies, provide staff education, and promote best practices.
  • Document care activities, participate in quality improvement, and ensure regulatory compliance.
  • Serve as a clinical resource, advocate for residents, and engage families in wound care education.
  • Maintain strong communication skills, proficiency with EHR and telemedicine tools, and physical ability to respond quickly in emergencies.
