Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is a full-time, in-person role for an APRN or PA working independently in PALTC facilities.
Manage comprehensive patient care by conducting exams, diagnosing, treating illnesses, prescribing medications, and providing preventive services. Work autonomously within scope, making clinical decisions and escalating as necessary, while collaborating with facility leadership and interdisciplinary teams. Ensure compliance with regulations, maintain accurate documentation, and participate in quality improvement initiatives. Promote health, safety, and infection prevention, educating staff and patients on wellness and disease prevention. Require at least 2 years of experience, board certification, and relevant degrees. Long-term care experience is preferred. Proficiency in EHR and telemedicine tools, strong communication skills, and the ability to work independently are essential.
