Focus Care Inc. is looking for experienced Nurse Practitioners to join our team in conducting in-home health assessments. This per diem position offers you the chance to work independently, with compensation for each assessment, plus mileage reimbursement.
As a Nurse Practitioner with Focus Care, you will:
* Conduct comprehensive health assessments in patients’ homes, including medical history reviews, physical exams, and screenings for chronic conditions.
* Provide education on medication management, disease processes, and preventive care.
* Identify and document diagnoses to support care management and active treatment.
* Collaborate with primary care providers to communicate findings and address potential gaps in patient care.
* Flexibly schedule your appointments throughout the week, with no on-call, weekends, or holidays required.
*Minimum Requirements:*
* Valid Nurse Practitioner license in the state of Maine .
* Board Certification from ANCC or AANP
* Minimum of 1 year of clinical experience; experience with geriatric populations preferred.
* Strong communication skills and a commitment to delivering compassionate care.
* Reliable transportation for travel within the assigned areas.
* Ability to manage various dwelling conditions and engage with patients comfortably in their home environments.
