Job Description

Seeking a dedicated Internal Medicine Nurse Practitioner for a full-time role in Syracuse, NY, within a hospital-based, post-acute primary care setting.

  • Provide comprehensive primary care to skilled nursing and long-term care patients of all ages, including physical exams, diagnoses, and treatment of acute and chronic conditions.

  • Collaborate with interdisciplinary teams to develop care plans, support quality improvement initiatives, and ensure high-quality, cost-effective care.

  • Utilize EMR systems for documentation, order and interpret diagnostics, and educate patients and families on health promotion and disease prevention.

  • Participate in utilization management and reviews to optimize healthcare resource use.

    • Qualifications include a Master’s degree in Nursing, NP certification, current licensure and DEA, proficiency with EMR, and a compassionate, team-oriented approach.
