Job Description

Seeking a dedicated Internal Medicine Nurse Practitioner in Syracuse, NY to provide patient-centered care in a post-acute, skilled nursing setting.

  • Deliver comprehensive primary care, including assessments, diagnosis, and treatment of acute and chronic illnesses.
  • Collaborate with interdisciplinary teams to develop care plans and improve quality initiatives.
  • Utilize EMRs to document and manage patient information effectively.
  • Order and interpret diagnostic tests, and educate patients and families on health promotion and disease prevention.
  • Participate in utilization management and reviews to ensure appropriate resource use.
