Job Description

The Nursing Home Wound Care Advanced Practitioner (AP) is a full-time clinical expert responsible for assessing, treating, and managing wounds in long-term care residents.

  • Perform comprehensive wound assessments and develop personalized care plans.
  • Administer advanced wound treatments, including debridement and negative pressure therapy.
  • Collaborate with multidisciplinary teams to ensure holistic wound prevention and care.
  • Implement evidence-based prevention strategies and provide staff education.
  • Maintain detailed documentation and participate in quality improvement activities.
  • Serve as a clinical resource, address infections, ensure regulatory compliance, and advocate for residents.
  • Educate residents and families about wound care and prevention.
  • Qualifications include a relevant degree, at least 2 years of experience, board certification, and preferred specialty certification in wound care.
