Job Description

The Nursing Home Wound Care Advanced Practitioner (AP) is a full-time clinical professional, such as a Nurse Practitioner or Physician Assistant with specialized wound management training.

They are responsible for

  • comprehensive wound assessments
  • developing and updating individualized care plans
  • performing advanced wound treatments
  • collaborating with multidisciplinary teams
  • implementing prevention strategies
  • educating staff, residents, and families
  • documenting care and participating in quality improvement activities
  • addressing infections and ensuring regulatory compliance
  • advocating for residents’ needs and preferences
  • using Electronic Health Records and telemedicine tools
  • possessing strong communication, leadership, and physical capabilities

  • Qualifications include a relevant degree, at least 2 years of experience, board certification, and preferred specialty certification in wound care.
